Athletic Facilities and Events

Completion of the Disability Needs Form will enable The Ohio State University to provide a reasonable accommodation to eligible patrons attending programs and events at Ohio State's athletic facilities based on both an individual's request and Ohio State's assessment of individual needs. On this form, you can indicate a need for accommodations including, but not limited to: 

•  Accessible (wheelchair) seating
•  Companion seating
•  Assistive listening device
•  Seating for visually impaired
•  Alternative communication aid or service

To complete the form you may:

• Fill in the online OSU Athletic Department Disability Needs Form
• Request disability accommodation by phone by calling The Ohio State University Athletic Ticket Office at 1-800-GOBUCKS (1-800-462-8257)
• Fill in a Microsoft Word version of the OSU Athletic Department Disability Needs Form and return it in person or by fax, email, or U.S. Mail to:
