Requesting Travel Accommodations
Employees and visitors should submit an accommodation request through the ADA Coordinator's Office. The request should include:

Employees are encouraged to submit requests as far in advance as possible before planned travel.
Required Documentation
Medical documentation may be requested when the disability or need for accommodation is not apparent. Documentation should:
- Confirm the existence of a disability and describe the nature of the medical condition, including relevant medical information related to the condition (e.g., symptoms, diagnosis, functional impact, and treatment regimen, as applicable).
- Describe the functional limitations that affect the individual's ability to participate in university-sponsored travel.
