Request Template

To request American Sign Language (ASL) Interpretation or Live Captioning (CART) services as an accommodation, please complete the following information and email it to ada-osu@osu.edu:

Once your request is submitted, we will confirm receipt and provide updates regarding interpreter or captioner availability.

Information About Requests for OSU Events

The ADA Coordinator’s Office is committed to supporting accessibility for your event. Central funding for accommodations such as ASL interpretation and CART may be available if the event meets the following criteria:

Free and Open to the Public
Includes an Accommodation Statement (see sample below)
An Event Participant Has Made a Specific Accommodation Request
Large or Disability-Focused Events: Events expecting more than 200 attendees or focused on Disability or Deaf culture

Events are typically not eligible for central funding if:

Accommodation Statement Is Absent: You may be responsible for covering costs
Revenue-Generating Events: Accommodation expenses should be included in the event budget
Student Requests: If the request is from a registered student, please contact Tiffany Hedges at hedges.10@osu.edu

The ADA Coordinator’s Office is happy to advise on accessibility strategies and services for your event, regardless of funding eligibility.

Sample Accommodation Statement

“If you have questions about accessibility or wish to request accommodations, please contact [Name] at [Email Address]. Typically, a two-week notice will allow us to provide seamless access.”

The contact person listed should know basic event details (start time, duration, location, parking, program overview). If contacted for ASL or captioning accommodations, they should complete and send event information to ada-osu@osu.edu.

If the contact person is unsure about the accommodation needed, they should collect the requester’s name and contact information and forward it to ada-osu@osu.edu or call 614-292-6207 for assistance.

Frequently Asked Questions

How Far in Advance Should I Submit a Request?

We recommend submitting requests at least 10 business days in advance. While we will make every effort to fulfill requests regardless of timing, additional notice is strongly encouraged for:

Events longer than three consecutive hours
Events outside standard business hours
High-volume periods (e.g., the first weeks of the semester, midterms, finals, commencement week)

Can I Submit One Email for a Series of Similar Requests?

Yes! For recurring needs (e.g., a semester-long class or regular meetings with the same participants or topics), you may submit a single email listing all known dates, times, and locations. Updates can be communicated within that same email thread.

Can I Request a Specific Interpreter?

If you have preferred interpreters or vendors, please include that information with your request. While we cannot guarantee availability, we will make every effort to accommodate your preferences.

What if I Need to Cancel or Reschedule the Request?

If you need to cancel a request, please provide at least 48 hours’ notice whenever possible. Changes to the date and/or time of a request are considered cancellations.

If you need to reschedule, please send the updated details. We will submit the new request to the vendor on your behalf.
